asked the Secretary for Mines whether he has received a report regarding Mr. Thomas E. Teale, of 34, Audrey Street, Ossett, who died oh 8th June following an accident which occurred at the Hartley Bank Colliery, Ossett, on 22nd May; will he make inquiries as to the cause of the accident; and whether negligence is attributable to any particular person?

From the inquiries already made and the report I have received, it appears that this regrettable accident happened while Mr. Teale and others were walking out along a mechanical haulage road after they had finished work. The haulage rope was not running when they set out, but it was started and run while they were on their way. The inquest verdict in this case was "death by misadventure." I understand, however, that the manager considered there had been failure to take proper measures to observe the rule which prohibits travelling on this haulage road while the rope is in motion, and that he has taken disciplinary action against the person whom he considered to be chiefly responsible.
