To ask the Secretary of State for Health what proportion of patients discharged from a medical specialty in each NHS trust in each of the last three years were subsequently readmitted as an emergency admission to that or another NHS trust within 28 days of discharge(a) as a crude percentage and (b) standardised for age and sex.
