Mr. Wigley

asked the Secretary of State for the Home Department if he will publish a table listing those statutory forms used by coroners that are (a) available in either a Welsh language or bilingual format and (b) only available in an English format.

The forms used by coroners, wind] are printed in the third schedule to the Coroners Rules 1953, as amended, are not prescribed forms in the usual sense. The coroner is not required to use them, but rule 42 of the 1953 rules provides that they may be used for the purposes expressed with such modifications as circumstances may require.

The forms are as follows:

  • Form of declaration of office of coroner.
  • Warrant to exhume.
  • Warrant to summon jury.
  • Summons to juror.
  • Notice to accompany summons and reply thereto.

  • Form of oath of juror.
  • Summons to witness.
  • Oath of witness.
  • Direction to medical practitioner to make a post-mortem examination.
  • Certificate of fine.
  • Form of recognizance-witnesses or jurors.
  • Condition.
  • Notice of inquest arrangements.
  • Notice that an inquest which is adjourned in pursuance of section 20 of the Coroners (Amendment) Act 1926 will not be resumed.
  • Notice that an inquest which is adjourned in pursuance of section 20 of the Coroners (Amendment) Act 1926 will be resumed.
  • Notice that the attendance of a witness will not be required at the holding of an adjourned inquest.
  • Notice that the date, time or place fixed for the holding of an adjourned inquest has been altered.
  • Certificate of forfeiture of recognizance.
  • Order to remove body for inquest or post-mortem examination.
  • Coroner's order for burial.
  • Inquisition.

The coroner's order for burial is available in a version which is partly in Welsh and partly in English. No Welsh language versions of the other forms have been prescribed.

We have received no recent requests to provide Welsh versions of any of the other forms but would consider sympathetically any such request we received.
