  1. (a) shall be nominated for appointment to a Family Practitioner Committee under sub-paragraph (3)(f) or (g) above or sub-paragraph (4) above; or
  2. (b) shall be appointed to such a Committee under sub-paragraph (3)(f),(g) or (h) above,
if he is—
  1. (i) a medical practitioner;
  2. (ii) a dental practitioner;
  3. (iii) an ophthalmic optician;
  4. (iv) a dispensing optician;
  5. (v) a registered pharmacist; or
