`In this Part of this Act "personal records" means documents and other records concerning an individual (whether living or dead) who can be identified from them, and relating—

  1. (a) to his physical or mental health;
  2. (b) to spiritual counselling or assistance given or to be given to him;
  3. (c) to social work or similiar activities involving counselling or assistance given or to be given to him; or
  4. (d) to other activities relating to his personal welfare and involving counselling or assistance given or to be given to him—
    1. (i) by any voluntary organisation; or
    2. 139
    3. (ii) by any individual who by reason of his office or occupation has responsibilities for his personal welfare.'.—[Mr. Whitelaw.]
