§ Mr. MASTERMANThe model form of application issued by the Commission for the assistance of approved societies, can be adapted by any such society to meet the requirements of their particular case. The way in which the question 5 (c) "How are you paid?" is answered must depend on the circumstances of the particular applicant, but in the cases to which the hon. Member refers sufficient information would be given by the statement "out of the profits of my business," taken in conjunction with the answer to Question 4, as to the exact occupation of applicant.
