§ Ms Glenda JacksonThe Health and Safety Executive's (HSE's) Railway Inspectorate commenced an independent investigation on the morning of 30 April, when this incident occurred. HSE's investigation is continuing and it is too early to speculate about the causes. HSE will make its findings and recommendations known in due course.
