§ Mr. JackFollowing the pilot of the single form to give the self-employed a single notification point for tax, national insurance contributions and VAT, the use of the form was extended to the whole of the country in April 1996. The form is available from tax, social security and VAT offices. The form is accompanied by a new leaflet which guides people commencing self-employment through the ins and outs of income tax, national insurance and VAT, explaining how to get things right first time. Both leaflet and form have been well received by customers.
