To ask the Secretary of State for Health how many general practitioner fundholding practices there were in each family health service authorities and regional health authority in each of the last four years.

To ask the Secretary of State for Health how many patients were de-registered from general practitioners lists in total and at the request of the general practitioner broken down by family health service authority, by regional health authority and separating fundholding from non-fundholding general practitioners in each of the last four years.
