To ask the Secretary of State for Wales, how many forms he has extended in respect of the numbers of questions requiring an answer, in a manner analogous to the extension to form PHS1 as provided for in guidance note FHSL [W] 50/94 of 16 December 1994 on local pharmacy budgets; and if he will make a statement. [holding answer 3 April 1995]: Of the 106 Welsh Office forms to the NHS reviewed in 1993–94 and 1994–95, additional information needs were identified on six occasions. Some 35 forms were' discontinued in the first phase of the review. On 28 March, I announced a further reduction of 30 per cent. on the information collected on remaining statistical returns.

Form PHS1 is issued by the Department of Health and is included in its rolling programme of information reviews.
