Mr. Anderson

asked the Secretary of State for Social Services what guidance he gives to general practitioners about signing forms on behalf of patients without an examination taking place, such as form OS.C.1, which requires the patient to be examined by an optician.

Guidance about the completion of individual forms is given when necessary. I have no evidence that general practitioners require any further advice.
