The fatality was later attributed to causes unrelated to the pump and the incident is not included here. 2The fatality listed here was reported to SHS on 8 June 1995. Two infusion pumps used to treat the patient were examined by SHS and a technical report is currently with the Procurator Fiscal. The fiscal has confirmed his conclusion that the fatality was not a result of over-infusion and this incident is not included in the column (b).
