Medical condition is based on evidence provided at the start of the claim, this in itself does not confer entitlement to ESA and may not represent a claimants most recent medical condition.</p><p>When the claimant makes a claim for ESA the contact centre records any health condition information that the claimant provides. The claimant then sends in evidence of incapacity, which will often include a fit note. This information is used to update the medical information that we hold. The health condition information is updated again if it changes during the course of the claim, sources of information could include further medical evidence, the health care professional report or the claimant informing the Department. Changes could include a change of diagnosis or identification of additional health conditions.
