§ Mr. JamiesonThe Health and Safety Executive have provided the following list of major railway accidents since 1992. A major accident has been defined as those where either the Health and Safety Commission directed the Health and Safety Executive to produce a special report or where the Commission (with the consent of the Secretary of State) directed a public inquiry.
