§ Mr. RichardsThe circumstances leading to this regrettable incident are being investigated by the University Hospital of Wales NHS trust and the coroner's office. The trust will examine whether proper procedures were adhered to and implement any necessary improvements to current systems.
Welsh Office officials will be kept informed to determine whether lessons learnt should be disseminated more widely.
