§ Mr. HanleyHMS Illustrious is at present undergoing a refit in Devonport dockyard. On the morning of 7 January 1994 an accident occurred when a member of the ship's company fell into the machinery of the vessel's vertiflo stores lift. He suffered severe facial injuries and was transferred to a local hospital.
An investigation was immediately initiated by the ship to establish the circumstances which led to the accident. The report is an internal document produced for internal use and there are no plans to publish it. However, a copy would be made available to the individual concerned on request.
