asked Her Majesty's Government:

How many Directives and Regulations have been approved by the European Community Council on the basis of majority vote since the coming into force of the Single European Act; and in which cases was the United Kingdom a minority.

Council voting records are not made public. In practice, actual votes are not always required. According to our information, the United Kingdom was in a minority in Council votes on three occasions when a qualified majority vote was taken under a treaty base changed by the Single European Act. Two of these were the same measures (common position and final adoption).
