asked the Secretary of State for Social Services whether his Department will take steps to ensure that doctors complete the name, address and age section for each patient at the top of each National Health Service form.

[pursuant to his reply, 13 November 1980, c. 428–29]: I have no evidence to suggest that doctors are completing NHS prescription forms without the patient's name and address—and age when required; but, if my hon. Friend would care to write to me, I shall be pleased to look into any particular point she has in mind.
