§ Mr. MayhewSufferers who were first awarded disablement benefit before the Act came into force on 4 July 1979 and dependants of deceased sufferers who died before then should send completed application forms to the Deparment of Employment, HSL 6, 12 St. James's Square, London SW1Y 4LL by 3 July this year. Blank forms and explanatory leaflets can be obtained from that address or from the Department's offices for Wales, Companies House, Crown Way, Maindy, Cardiff SF4 3WW, or office for Scotland, Pentland House, 47 Robb's Loan, Edinburgh EH14 1WE.
