§ Mr. Charles R. MorrisMy Department does not supply forms requiring completion by members of the general public, except in relation to specific applications, for example, for appointment. In such cases a Design and Control Unit in my Department ensures that the information sought is essential for the application in question, and that the content of the form is clear.
