asked the Secretary of State for Social Services why, in redesigning application form JDS2, for attendance allowance, question 10, which in a previous edition of the form, asked for any additional useful information which could be used in deciding the claim, has been omitted completely and not replaced.

Claimants were invited to state the disabled person's attendance needs on the original claim form, DS2, because this was the only formal opportunity claimants had for making a signed statement before a decision was reached. When the attendance allowance medical report forms were revised provision was made for the claimant to make a signed statement of the attendance needs in much more detail on the front of these forms. The question on the claim form became unnecessary, therefore, and the opportunity was taken to omit it when the claim form was recently revised and reprinted as a tear-off portion of the attendance allowance leaflet NI 205.
