§ Mr. Hayasked the Secretary of State for Foreign Affairs if he will amend the notes on the form of application for a passport, to make it clear that where the applicant wishes the passport to be endorsed in respect of his children, birth certificates of the children must be submitted with the application.
§ Commander NobleYes. When the application form "A" is reprinted, Note 1 will be amended to make it clear that birth certificates must be produced when children are to be included in a parent's passport.
