§ Mr. PeakeThe forms of application for welfare milk and vitamin tokens ask only for essential information to enable a mother to show that she is entitled to these benefits for her children and to satisfy the Department that a duplicate claim is not being made. The forms are kept under constant review and have been simplified this year.
