asked the Secretary of State for Social Services if he intends to revise the new form MED 3 in order to preserve confidentiality.

There are no plans to revise new form MED 3, but guidance issued by the Department to general medical practitioners explains the procedure that a doctor may use if he wishes to avoid entering the real diagnosis on the form.
