§ Mr. HoramSince January 1988, when the Medical Devices Agency's computerised database started, there have been 705 adverse incident reports involving infusion devices; 236 of these reported possible over-infusion. In the same period, there were 23 reported fatalities involving infusion devices. Between August 1993 and November 1994 there were six reported fatalities involving infusion devices. Since August 1993 there have been 18 reported fatalities involving infusion devices.
Information prior to January 1988 could be provided only at disproportionate cost.
