§ Mr. BayleyTo ask the Secretary of State for Health what proportion of patients discharged from hospital in(a) England, (b) each regional health authority and (c) each hospital or trust between January and March had a community care plan for their continuing health and social care needs agreed with them prior to discharge.
§ Mr. BowisThe patients charter states that before a patient is discharged from hospital a decision should be made about any continuing health or social care needed. Local authorities and health authorities should work together to assess and respond to the needs of people who may need long term care and should agree local arrangements for monitoring hospital discharge.
