(2) how much has been paid from public funds, in real terms to limited companies set up by general practitioner fund holders (a) individually and (b) in total in the years 1991–92 and 1992–93;
(3) if she will list the names of general practitioner fund holders who have been invited to return money from the limited companies to the national health service or risk losing their fund-holding status; which of them have been asked to return sums (a) between £50,000 and £100,000 and (b) over £100,000; and if she will make a statement.
