What international institution or agreement exists to bring to justice those guilty of war crimes in the Gulf war and in present hostilities in the Balkans.

None. Grave breaches of the Geneva Conventions can, in theory, be prosecuted in the national courts of any state party to the conventions; in the United Kingdom prosecutions could be carried out under the Geneva Conventions Act 1957.
