§ Mr. Gwilym JonesForms AG1 are supplied to family health services authorities in Wales which are responsible for its distribution to the general ophthalmic, dental and medical services. Adequate stocks are maintained centrally to meet all orders for these forms.
The form has recently been revised and the new version has been issued to all FHSAs in Wales.
