§ Mr. John SmithI am advised that my Department requires the completion of 43 statistical returns, some of which are mandatory under statute or regulation, and the following further 13 forms:
- Application under the Energy Saving Loan Scheme.
- Application for Offshore Supplies Office Interest Relief Grant.
- Application for Registration of Offshore Installation.
- Notification of change of location of Offshore Installation.
- Notice of appointment of manager of Offshore Installation.
- Notice of termination of manager of Offshore Installation.
- Notification of death or loss of a person from Offshore Installation.
- Application to become an approved Doctor for diving.
- Certificate of medical fitness for diving. North Sea incident report note.
- Application for exploration/production licence.
- Log Book to be kept by Offshore Installation.
Information is not available within my Department in respect of forms required by the energy nationalised industries.
