§ Sir K. JosephThe information which one doctor gives to another about the condition and earlier treatment of a patient is a matter for his professional judgment. My Department has, with professional advice, since 1965 made available and encouraged the use of certain standard medical record forms, including forms for use when a patient is referred by a general practitioner to a hospital and when a patient is discharged from hospital; this system is kept under review.
