§ Sir A. GEDDESNo, Sir; I had not seen the form until my attention was directed to it by this question. It appears to have been drafted in 1915 by the Parliamentary Recruiting Advisory Committee and to have been in use in Manchester since that date. There is no compulsion on anyone to fill up the form, but if anyone has no objection to doing so it would appear to help his case and save time.
